Maylard type incision approach in Gynecology.

The most used incision in the gynecological area is the Pfannenstiel incision but it does not provide adequate abdominal exposure, the midline incision being the most used for abdominal surgery, it is not very aesthetic, unlike the Maylard incision, the transverse incision which provides advantages; as a better abdominal, retroperitoneal, pelvic exposure and with satisfactory aesthetic results. The objective of this work is to demonstrate the results and uses of the Maylard incision in gynecological oncology in 160 patients who underwent surgery at the La Trinidad Medical Teaching Center and the Center Medico La Floresta during the period from January 2017 to December 2023. Results of the 160 patients, 66 ovarian protocol, 37 endometrial protocol, 28 radical hysterectomy, 16 interval surgery, 3 rectus vaginal fistulas, 3 appendix tumor, were identified. 2 uterine sarcomas, 1 small intestine tumor, 1 retoperitoneal tumor, 1 vaginal cancer, 1 total colectomy, 1 primary peritoneum tumor. Conclusions. Optimal pelvic exposure is necessary. Which speed of entry and access to the upper abdomen are not critical factors for the Maylard incision. It is the surgical incision that provides the best postoperative and aesthetic results in patients with gynecological oncological pathologies
