Stromal tumors of the sexual cords correspond to 5 to 8% of all malignant neoplasms of the ovary. They are the third most common type of ovarian tumor after epithelial and germ cell tumors. Thecomas are uncommon neoplasms, which represent 1 to 2% of ovarian tumors and can appear at any age, but the majority do so in the menopausal period. They are usually unilateral, solid, of variable volume, smooth surface and well encapsulated. The prognosis is generally good. The following clinical case is presented of a 60-year-old patient with a history of abdominal hysterectomy plus adnexa conservation due to endometrial ADC, who is referred to the Gynecology Oncology surgery clinic due to pathological findings in the tomography, the left ovary with a diameter of 40 x 39 mm, with a hypodense image in the fluid range, rounded, circumscribed that does not present contrast material uptake, with a diameter of 30 mm and peripheral punctate calcification. To conclude, ovarian stromal tumor, thecoma type, is a rare tumor that can be hormonally active and predispose to developing endometrial carcinoma, as is the case of the patient and her history of endometrial ADC.
