Smooth muscle neoplasms of the uterus are very varied. Leiomyomas, however, continue to be the most common. It is important to recognize some variants of these, as they can be misinterpreted as leiomyosarcomas. The objective of this study is to present a clinical case of a patient whose histopathological result shows leiomyoma with bizarre nuclei. Leiomyoma with bizarre nuclei is characterized by large cells diffusely, focally, or multifocally distributed in the background of a typical leiomyoma. These large cells are oddly shaped, multinucleated or multilobulated, and have eosinophilic cytoplasm. Leiomyomas with bizarre nuclei are a rare variant, diagnosed as a casual finding, so there are few studies about their behavior and surveillance.
