<jats:p>Los procedimientos quirúrgicos realizados en la mandíbula requieren un conocimiento detallado de la posición y trayecto del canal alveolar inferior (CAI). La odontectomía de terceros molares inferiores es uno de los procedimientos más frecuentes en cirugía oral, aunque presenta alta variabilidad de complicaciones debido a su estrecha relación con el nervio alveolar inferior. La tomografía computarizada de haz cónico (TCHC) se ha consolidado como una herramienta de gran valor diagnóstico, ya que permite analizar tridimensionalmente la proximidad entre las raíces de los terceros molares y el canal mandibular, superando las limitaciones de la radiografía panorámica. Objetivo: Determinar la asociación entre el canal mandibular y las raíces de los terceros molares mediante TCHC. Material y métodos: Estudio observacional, transversal, retrospectivo, descriptivo y correlacional. Se analizaron 32 tomografías seleccionadas de una base de 1.155 exámenes del Departamento de Odontología del Centro Médico Docente La Trinidad 2024. Se evaluaron el estadio radicular según Nolla, la clase y posición de Pell y Gregory, y la distancia nervio-raíz en cortes axial, coronal y sagital. Resultados: Predominó el estadio 10 de Nolla. La Clase III, Posición C presentó el mayor contacto con el CAI, con una distancia promedio de 0 mm, mientras que la Clase II, Posición B mostró menor incidencia de contacto. Conclusiones: La TCHC es un método diagnóstico esencial para valorar la relación entre las raíces de los terceros molares y el CAI. En casos de contacto directo, la coronectomía representa un abordaje seguro que reduce el riesgo de complicaciones neurosensoriales.</jats:p>
